The aesthetic plan menopause never got

If you are in your mid-forties or your fifties and your face has started behaving differently, you are not imagining it. 

You wake up looking tired even when you’ve slept. The skin around your jaw feels softer than it used to. Lines that used to settle when you stopped frowning now stay etched in. Your skin drinks moisturiser like it’s never seen water before. The makeup that worked last year now sits in places it didn’t used to sit. 

This is not just ageing. This is oestrogen.

What menopause actually does to your face 

The fall in oestrogen during perimenopause and menopause changes four things at once, and they all change at the same time. 

The first is collagen. We lose around 30 per cent of skin collagen in the first five years after menopause. That’s a structural change, not a cosmetic one. Skin thins. Elasticity drops. Lines etch in faster and stay longer. 

The second is hydration. Oestrogen helps the skin hold water. When it falls, the skin barrier weakens and the dermis loses hyaluronic acid. Skin that used to look plump looks tired. 

The third is bone. The jawbone resorbs. The cheekbones flatten subtly. The chin loses projection. This is the change most patients can feel but can’t quite name. The face’s underlying scaffolding is shifting, and the soft tissue on top has nowhere to sit. 

The fourth is the skin barrier itself. Sensitivity goes up. Redness goes up. Things that never used to irritate your skin suddenly do. 

And here is the problem with the standard aesthetic-clinic response to all of this. Some toxin in the forehead and a syringe of cheek filler does not touch most of what is actually happening. You can fill the cheek, but if the underlying bone has gone and the skin quality is poor, the filler looks placed. It looks like filler. It doesn’t look like you. 

Menopausal faces need a different conversation.

What a layered plan actually looks like 

For my menopausal patients I write plans that work on multiple layers of the face at once, staged over months. There is no single treatment that does what menopause has done. There is a sequence. 

  • Bio-remodellers. Profhilo, Sunekos, sometimes Sculptra or Radiesse as biostimulators. These are not fillers in the traditional sense. They work in the dermis to stimulate the skin’s own collagen and elastin. For thinning, dehydrated, post-menopausal skin, this is foundational. A two-session Profhilo course gives around six months of dermal improvement, and we maintain it from there. 
  • Polynucleotides. Plinest, Ameela, NCTF. These work at the level of the fibroblast — the cells that produce collagen — and improve skin quality, hydration, and around the eye area especially. A course of two to three sessions, three to four weeks apart. Maintenance every six months. The periorbital improvement in particular is something I cannot replicate with any other treatment. 
  • Conservative filler, where structure has been lost. Not to chase volume. To replace it where bone has resorbed. Cheek and chin projection, jawline definition. Hyaluronic acid only, because it is reversible. Anatomy-led, conservative volumes. I would rather underfill twice than overfill once. 
  • Toxin where it earns its place. Targeted, micro-dosed, often into the lower face for masseter and platysmal banding as much as the upper face for frown and forehead. Treating muscles in harmony, never one area in isolation. 
  • Skincare alongside everything. Vitamin A, vitamin C, sunscreen, barrier support. Without the skincare, the in-clinic work underperforms. With it, the results last longer and look better. 
  • HRT, where appropriate. Your GP prescribes it, not me. But the skin’s response to HRT is part of the same conversation, and I’ll always ask whether you’ve discussed it with your GP if I think it would help.

Why this matters 

Most aesthetic clinics will sell you single procedures forever and you will keep feeling like you’re chasing something. That’s because you are. You’re filling above an empty foundation, smoothing skin that isn’t being supported, treating animation lines on a face that has lost dermal collagen. 

The work that actually serves a menopausal face is layered, staged, and patient. It takes months, not minutes. The results build slowly. And they look like you, rested, rather than like someone who has been treated.

A treatment plan, not a single visit
